Immunization forms need to be in to the nurse's office by Monday, February 9th!

Minnesota law requires students to be vaccinated unless there is a medical reason or because of the conscientiously held belief of the parents. 
 You will be receiving another letter home by US mail if your student is missing immunizations. Proof of immunizations or a notarized note for medical or conscientious objection must be received in the health office by February 9 or students will be excluded from school.
If your child has medical assistance or no insurance, they can receive their immunizations in the school based clinic at school with a signed parent consent. The school based clinic can provide all immunizations except for varicella. Please make an appointment at the school based clinic or your child’s health care provider if your child is lacking immunizations and have your student bring proof back to the school health office in room 212 by February 9th
