Immunization Records

Our MPS School Based Clinic (SBC) is conducting outreach to families regarding students who have incomplete immunization records. Most of these immunizations can be given through the SBC at PH M-F from 9am to 2pm. Students need to have a consent form signed by parents. If you have received a letter, or believe that your student is in need please contact Kathy Vang at 612-668-1944 or call/text 612-449-1821.

Minnesota law requires that in addition to the shorts required in other grades, all students entering 8-12th grade must have received vaccination against Hepatitus B, (a series of 3 shots), Varicella (2 doses unless they have had chicken pox), measles, mumps and rubella (2 doses of MMR), tetnus, diphtheria and pertussis 1 dose of Tdap or Td, and meningitis (2 doses one after 10th birthday unless they claim an exemption. If the student should not have the shots for medical reasons or because of the conscientiously held beliefs of the parents, the school must have written proof of exemption.
